PC Acquisition (login required)

The PC acquisition form is for requesting new computers for new positions. In order to expedite your request, please provide us with as much information as possible. Allow three to five days for processing. If you have any questions contact the Service Desk at extension 3460.

PC Transfer (login required)

The PC transfer request form is for requesting computers and/or computer equipment to be moved from one place to another. In order to expedite your request, please provide us with as much information as possible. Allow three to five business days for processing. If you have any questions contact the Service Desk at extension 3460.

PC Redeploy (login required)

The PC redeploy form is for requesting surplus computers for other uses. In order to expedite your request, please provide us with as much information as possible. Allow three to five days for processing. If you have any questions contact the Service Desk at extension 3460.

Software Installation (login required)

The software installation request form is for requesting software to be installed on lab computers only. In order to expedite your request, please provide us with as much information as possible. Allow ten business days for processing. If you have any questions contact the Service Desk at extension 3460.

Event Technology (login required)

This request form will allow you to request Technology for an Event on any campus. This form can also be used to request a demonstration or tutorial of the technology equipment.

Banner Account (login required)

This form is to request Banner access for new users only.  The department Budget Manager must sign in with his/her GPC email username and password.  A GPC email account is required for the employee in order to request Banner Access.

Requests by the Budget Manager to add or remove access from a current Banner user must be emailed to the appropriate person below.

Include the Banner user’s Name, Username, and classes to be added and/or removed.

Guest Account (login required)

Please login here with your credentials in order to access the form to place an account request for your guest. The access can remain active for up to three days upon activation. This form requires a two business day prior notice and a budget manager, department head/supervisor notification. Please check the form for accuracy as errors/ommitions can cause a delay in provideing the service. Please also keep in mind that this form does not grant the guest a GPC email account – it’s for network access only.

  • Sponsor’s name
  • Department Head/Supervisor name
  • Guest’s first and last name
  • Day or date range for requested access

Those who need network access AND a GPC email account, such as a long-term consultant or contractor, should use the GPC Consultant/Contractor E-mail ID Request Form. However, before the consultant/contractor fills that form out, their supervisor would need to contact Doug Ruch to get a GPCID (900#) created.

Consultant/Contractor Email ID Request

This form is for Consultants/Contractors Only. Students should go to GetMyLogin to obtain their login ID’s and initial passwords.
